Office Hours 

Our office opens at 9:00 a.m. and closes at 4:30 p.m. Monday through Thursday. The office is closed daily for lunch and every Friday.  Please plan to leave a voice message when calling the office, as Dr. Knezek is routinely in session and unable to answer incoming phone calls immediately. Voice messages are checked periodically. All messages and communication initiated after 4:30 p.m. Monday through Wednesday will be retrieved the next business day. All messages and communication initiated after 4:30 p.m. on Thursday will be retrieved the following Monday, excluding holidays.  

If you experience a life-threatening emergency, please call 911. For other emergencies, you may contact Dr. Knezek by calling 214-738-4956.  

Insurance and Payments  

Payment in full at the time of service is required. Payments may be made by cash, check, Visa, MasterCard or Discover.  

Dr. Knezek does not accept insurance. A statement is provided that can be used to file with your insurance for reimbursement if you choose to do so.  

We require a credit card on file (Visa, MasterCard, or Discover) to reserve your appointment time. Your card will be charged if you do not show up for your appointment, or if you give less than 24 hours’ notice rescheduling a follow-up appointment / 72 hours’ notice rescheduling a new patient consultation when it is not a true emergency. Monday appointments must be rescheduled by noon the preceding Thursday.  

Scheduled Appointments 

Patient sessions usually begin at 9:00 a.m., with the last appointment scheduled to begin at 4:30 p.m. Occasionally, Dr. Knezek will make an exception and permit scheduling patients outside of these standard operating hours.  

To provide the best care for his patients, Dr. Knezek requires follow-up appointments of at least 30-minutes every 6 months (biannual requirement).  

Please understand that occasionally the person(s) being seen before you may need additional time that was not accounted for in the schedule. Dr. Knezek provides the additional attention and care needed to assist the person in session. 

Reminder Calls and Emails 

Automated courtesy appointment reminders are available and are disseminated one or two days prior to scheduled appointments. It is each patient’s responsibility to remember and attend their scheduled session even if an automated courtesy appointment reminder is not received. 

It is the patient’s responsibility to ensure all contact information is current using the Patient Portal to facilitate receiving automated courtesy appointment reminders. Patients can view their upcoming appointments using the Patient Portal, sans appointments over 6 months from login date.  

Prescription Refill Requests  

Allow two business days for prescription medication refill request processing. Please remember Dr. Knezek does not work on Fridays. Dr. Knezek does not accept faxed prescription medication requests. Only prescription medication requests received electronically through eRx systems from a pharmacy are accepted. Portal messaging is acceptable for requesting ADHD medication refills.

Prescription medication refills should be initiated by patients through their pharmacy first unless the prescription is for ADHD medication. Starting with your pharmacy first reduces errors, avoids delays, allows for much faster processing and ultimately, provides better results.

Refills for ADHD, controlled substance schedule II (C2) medication including Adderall™, Concerta™, Focalin™, Ritalin™, Vyvanse™, etc., and the generic equivalent of each, should be requested via portal messaging to Dr. Knezek, because current federal regulations prevent pharmacies from initiating eRx C2 medication refill requests. Contact Dr. Knezek if you encounter issues with your pharmacy request.  

If you are travelling, please request the area pharmacy send an electronic prescription request to Dr. Knezek to avoid having your medication filled at your primary pharmacy by mistake. Prescription medication requests may encounter delays for various reasons. Please plan accordingly, especially around holidays and before travelling.

Requests received after 4:30 p.m. Monday through Wednesday begin processing the following business day. Prescription medication refill requests received after 4:30 p.m. on Thursday begin processing the following Monday, excluding holidays.  

 Urgent prescription medication requests (requesting medication availability within 12 hours) which includes refills, will incur a charge for expedited / rush processing at the rate disclosed in the Fee Schedule. This fee will be charged at the onset using the patient’s card-on-file. Results are not guaranteed, and refunds are not available. Please use an automated reminder system, such as a recurring calendar event to prevent urgent prescription medication requests.  

If your prescription is not available at your pharmacy and a reasonable amount of time has transpired since your initial request, please verify with your pharmacy that an insurance hold has not been placed on your request.

Current Fee Schedule

SESSION FEES for both phone and in-person

  1. Follow-up appointment (not to exceed 30 minutes) – $300
  2. Follow-up extended appointment (not to exceed 45 minutes) – $450
  3. Follow-up interim brief appointment (not to exceed 15 minutes) – $150
  4. New patient psychiatric evaluation – $600

SERVICE FEES including phone and in-person

  1. Late cancellation (less than 24 hours prior to a follow-up appointment / 72 hours prior to a new patient evaluation); Monday appointments must receive notification by noon on the preceding Thursday – Fee identical to the cost associated with the time reserved for the session
  2. No-shows and non-emergency missed follow-up appointments – Fee identical to the cost associated with the time reserved for the session
  3. Rush prescription processing (requesting medication availability within 12 hours) for controlled, non-controlled, new, and refills – $100
  4. Medical testimony / deposition per hour – $900 (paid 72 hours in advance of reserved time)
  5. Non-sufficient Funds (NSF) check fee – $50
  6. Electronic Medical Records Release to Third-Party – $75
  7. Records Release with Brief Medical Summary Letter – $150
